There are steps to be admitted to long term care facilities that each new resident must take. 
1. Speak with your health care provider:

  • If you're in the hospital your doctor is recommending skilled nursing, rehabilitation, or long term care, communicate your choice of facility to your case manager or social worker planning your discharge.
  • If you're coming from home or assisted living, make an appointment to visit your primary doctor. 

We need updated health information from the last 10 days to get an accurate snapshot of your health situation.

2. ​Speak with your family. 

  • ​​Get a Power of Attorney for Health and Property if you don't already have one.
  • Let them know your wishes.
  • Encourage them to tour or check out our website.
  • Ask them to help you gather necessary items like clothing, toiletries, and any entertainment devices you may want  (radio, tablet, laptop).
  • Ask them to help you manage your affairs and change your address if needed.


3. We'll Evaluate Your Information

  • As with any facility, not every potential resident is a great fit. We'll need to check your medical status, compliance with treatment, and criminal background.
  • If you have private insurance, we may need to ask them for authorization to accept you.


4. ​Transfer Date

  • We'll set a transfer date to bring you from home or facility to us.
  • Transfer depends on your health. Some people can have family take them. Others get transferred by van or ambulance.


​5. Meet the Team

  • Our team will be anxious to meet you when you get here.
  • Make sure to let them know about any preferences for food, activities, or anything else we can do to make your stay more comfortable.
