  • You must have had a recent doctor's visit (within 10 days of admission).
  • Call the Admission's Director at (815) 235-6173 to make an appointment for a tour.
    • ​We will discuss your nursing home needs.
    • We will discuss how to pay for your stay.

      • Medicaid (we can help you apply)
      • Medicare Part B benefits for therapy.
      • Veteran's Benefits
  • ​​​We'll ask your doctor's office for information.​
    • ​History & Physical
    • Doctor's Orders
  • ​​We will review your medical information & evaluate you for appropriate placement.
    • ​If we accept you for admission, you or your representative will be contacted.
    • We will set a move in date/time.​
  • ​You will want to bring:
    • ​At least 6 changes of clothing including undergarments.
    • A couple pairs of pajamas if you'd like. 
    • Any preferred toiletries (we have the basics).
    • You can choose to bring a television and have cable installed to the room.
    • ​Insurance cards for any health or prescription insurance you have.​​
  • ​You or your representative will sign a contract on your day of admission.
  • It is strongly recommended that you have a Power of Attorney for Financial & Healthcare decisions. 

Civil Rights Policy

The Stephenson Nursing Center complies with Section 001 of the Title VI of the Civil Rights Act of 1964 which provides that:


"No person in the United States, shall on the ground of race, color, or nation origin be excluded from participation in, be denied the benefits of, or be subjected to discrimination under any program or activity receiving federal financial assistance. The Stephenson Nursing Center also complies with Section 504 of the 1973 Rehabilitation Act on that we do not discriminate on the basis of handicap, in accordance with Paragraph 84.8 (A) of the regulations."

IF YOU'VE BEEN IN THE HOSPITAL


Medicare recipients may qualify for benefits. 

  • You must have been admitted into the hospital (not just in observation) for 3 nights within the last 30 days. 
  • You must have a doctor give orders for skilled nursing or rehabilitation for the same reason you were admitted to the hospital.
  • Medicare pays for semi-private occupancy unless you have
  • Tell your hospital social worker that you prefer to go to Stephenson Nursing Center.
  • Your social worker will call to send us your recent information.
    • We will review your information for proper admission. 
    • We will contact the social worker to confirm your acceptance.
    • We may contact you or your representative for more information.
  • We will coordinate with hospital staff & your family to transfer you here.
    • We get your prescriptions transferred.
    • We will get your doctor's orders.
    • We will coordinate with hospital and family to get you transported.


You will want to bring:
